Prerequisites:

• Between the ages of 8-14
• Be able to push a small or children's size shopping cart
• Demonstrate independence
• Use Lionville, Gateway, or Paoli Pharmacy as family pharmacy
• Parents sign to the terms and conditions and consent forms

TOY Session Schedule (time limit: 90 mins)

1. Pair up with a senior (buddy)
2. Help your buddy shop around the store as needed with our TOY cart found at the store entrance
3. Visit the TOY cafe for snacks and refreshments with your buddy
4. Spend time with your buddy with activities found at the TOY center (ex. reading, coloring, or doing a puzzle)

Instructions

1. Fill out the online application below
• Include any allergies, requests, or concerns in the message box below
2. Wait to hear back from a pharmacy member after we run a background check and confirm your child's eligibility
3. Once you have heard back from us, watch the training video and frequently asked questions video
4. visit the pharmacy to sign the consent form and identiy confirmation.
5. You are all set to go please enjoy your time at the pharmacy.
